    Conoscenza del ruolo di fattori genetici in patologie umane, sia mendeliane che multifattoriali.

    Obiettivi formativi

    Al termine del corso, lo studente avrà acquisito la necessaria competenza per l’indicazione dei test genetici appropriati e per l’interpretazione dei loro risultati sia in ambito di consulenza prenatale che postnatale. Inoltre, lo studente dovra’ maturare le conoscenze adeguate per riconoscere le principali malattie genetiche e sindromi complesse.

    Lo studente deve aver superato gli esami del quarto e quinto anno

    Metodologie didattiche

    Lezioni frontali e lavoro interattivo con gli studenti su questioni aperte.
    Le tematiche di genetica clinica saranno corredate con presentazioni di casi clinici, in immagini e video

    Metodi di valutazione

    Esame orale.
    L’esame è basato su due domande. Sarà richiesta la conoscenza adeguata delle malattie genetiche trattate a lezione, dei test diagnostici molecolari a supporto e di un meccanismo/ragionamento di genetica medica volto a dimostrare la comprensione dei percorsi logici. Risposte non date, fuori tema o generiche saranno considerate elementi di non sufficienza. La lode sarà proposta collegialmente per gli studenti che dimostreranno una conoscenza oltre quella richiesta dal corso.

    Programma del corso

    1. I test genetici. L'analisi del cariotipo e i bandeggi, la FISH. Array CGH. PCR, sequenziamento, real-time PCR, MLPA. Analisi di linkage, microsatelliti, CNV e SNPs. Next generation sequencing (NGS), WGS, WES, targeted NGS. Test prenatali invasive e test prenatali non invasivi, incluso il Noninvasive Prenatal Testing (NIPT)
    2. I meccanismi di trasmissione ereditaria dei caratteri mendeliani. Eredità mitocondriale. L’imprinting e la disomia uniparentale. Eterogeneità clinica ed eterogeneità genetica. Penetranza ed espressività, anticipazione. Omozigosità ed eterozigosità composta. Aploinsufficienza.
    3. Forme mendeliane di malattie neurodegenerative
    4. Forme di cancro ereditarie
    5. Malattie cromosomiche e subcromosomiche
    6. La consulenza genetica in ambito postnatale e prenatale. Screening neonatale.
    7. Caratteri multifattoriali e studi GWAs
    8. Principi generali di terapie avanzate per malattie genetiche (terapia genica, sostituzione enzimatica, terapia cellulare, editing genomico).

    Course objectives

    At the end of the course, the student will acquire the knowledge necessary to recommend the appropriate genetic tests and to interpret the related results both in prenatal and postnatal counseling. The student will also acquire the appropriate knowledge to recognize the main genetic diseases and complex syndromes.

    Prerequisites

    Student must have already passed All the 4th and 5th year exams

    Teaching methods

    Lectures and interactive work with students on open issues. Work in small groups.
    The topics of clinical genetics will be accompanied by presentations of clinical cases, in images and videos

    Evaluation methods

    Oral examination. The oral exam is based on two questions. Appropriate knowledge of genetic diseases treated in class, of supporting molecular diagnostic tests and of a mechanism / reasoning of medical genetics will be required to demonstrate the understanding of logical pathways. Answers not given, off topic or generic knowledge will be considered as elements of insufficiency. The praise will be proposed collegially for students who will demonstrate a knowledge beyond that required by the course.

Next generation sequencing (NGS), Whole Genome Sequencing (WGS), Whole Exome Sequencing (WES), targeted NGS. Invasive prenatal tests. Non-invasive Prenatal tests, including Non-invasive Prenatal Testing (NIPT)
    2. The mechanisms of human genetics. Transmission of Mendelian characters. Mitochondrial inheritance. Imprinting and uniparental disomy, diseases caused by dynamic mutations. Clinical heterogeneity and genetic heterogeneity. Penetrance and expressivity, anticipation. Homozygosity and compound heterozygosity. Haploinsufficiency.
    3. Mendelian forms of neurodegenerations
    4. Hereditary cancer
    5. Chromosomal and subchromosomal diseases
    6. Genetic counseling, both postnatal and prenatal. Newborn screening
    7. Multi-factorial characters and GWAs studies
    8. General principles of advanced therapies for genetic diseases (gene therapy, enzyme replacement, stem cell therapy, genome editing)
